JOB SUMMARY:

Scheduled: FT 40 hours/week
Under the guidance of the Nurse Manager the LPN is responsible for the provision of patient
assessment, plan-of-care, implementation of plan, and the evaluation of patient care utilizing the
nursing process to ensure quality patient-care. The LPN is responsible for carrying out duties as
assigned and delivers care to patients in accordance with NHSC policy. The StaffRJf constructs
and updates a Nursing plan of care in conjunction with the medical plan of care that reflects
comprehensive assessment of the patient and documents all Nurse patient interaction utilizing the
"SOAP" format. Utilizing a case management approach to care, the LPN evaluates the
effectiveness of nursing case and updates the nursing plan as indicated. The LPN also
participates in quality assurance activities as directed and administers
medications/treatments/immunizations prescribed by the physiciai1 in accordance with policy and
procedure. The LPN is responsible for continuing education and awareness of changing practices
within the profession.

1. Uses the nursing process and guidelines to correctly complete and document relevant
patient assessment on site
2. Promotes the family centered approach to patient case.
3. Documents assessment via computer/chart.
4. Applies knowledge of growth and development over the life span.
5. Ensures appropriate refenals and follow-up.
6. Provides telephone/walk-ins t1iage in accordance with NHSC guidelines and policy.
7. Assesses need for patient/family education.
8. Utilizes the Child & Geriatric Abuse Policy in the Assessment process.
9. Plai1s the nursing intervention of patients, demonstrates an understanding of the impact
on family and community along with interdisciplinary team.
10. Documents plans, goals, and patient outcomes.
11. Utilizes available resources in planning of case.
i 2. Participates in the establishment of department goals in the education
of patients and their families.
13. Evaluates and updates plan of care.
14. Executes physician orders in accordance with nursing standards and NHSC policies
and procedures, and documents immunizations appropriately.
15. Achninisters ordered medications in accordance with NHSC policy and
procedme.
16. Communicates appropriately with patients, families, peers and staff.
17. Negotiates the plan of care with patient and family.
