About The Position
 We are seeking a dedicated Nurse Practitioner to join our nephrology team, working alongside a group of experienced nephrologists to care for patients with chronic kidney disease (CKD). In this role, you will see patients independently, typically alternating visits with a nephrologist, with the goal of helping patients maintain their overall health and preserve kidney function.


For patients with progressive kidney disease, you will play a key role in educating them about renal replacement options, including dialysis and transplant, and preparing them for the next steps in their treatment journey. Your focus will be on providing compassionate care, helping patients navigate their treatment plans, and supporting them in managing their condition.


Essential responsibilities



  • Functions independently to perform comprehensive care for patients with chronic kidney disease.

  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.

  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities

  • Implements interventions to support the patient to regain or maintain physiologic stability

  • Monitors the effectiveness of interventions.

  • Collaborates with physicians by making appropriate referrals.

  • Facilitates patient and family decision making by providing educational tools.


Minimum qualifications



  • Graduate of an accredited school of nursing

  • Graduate of an accredited Nurse Practitioner Program

  • Current Advanced Practice Registered Nurse (APRN) Licensure

  • Current Nurse Practitioner certification

  • Strong computer skills,

  • Eligible for hospital privileges

  • Spanish speaking is REQUIRED for this role


 


Pay Range: $110,000/annually, and up, depending on experience
