Overview






The Nurse Manager - Respite/Bridge will be part of a collaborative, multidisciplinary team that will support the development, performance, and management of clinical patient care for nursing and medical assistants in a Medical Respite or Bridge Clinic Setting. The Medical Respite program allows individuals who are experiencing homelessness a safe place to rest and recuperative from a medical condition. While participants focus on improving their health, they can also work on addressing other needs like housing and benefits. The Bridge Program provides access to healthcare in a clinic setting and focuses on reaching people outside the clinic, meeting people where they are delivering needed health care in areas in our community.


This position will work as part of triad leadership team in making decisions that best support clinical staff, extended care team members and providers to ensure patients obtain access to care that is timely, effective, efficient, and safe. The Nurse Manager - Medical Respite/Bridge may work onsite at a primary location, or multiple locations based on business and patient needs.









Responsibilities






  • Supports the implementation of service delivery programs for Respite, Bridge, and Mobile Healthcare Services, collaborating with internal departments and external organizations to develop and coordinate programs and
    activities.
  • Collaborates with the patient’s care team including primary care team, specialists, home care, hospital team, and any others involved with the patient’s care to optimize clinical outcomes.
  • Performs regular analysis of key performance indicators to improve clinical care workflows that lead to improvement of the quality of patient care and enhance cost effectiveness.
  • As needed, conducts chart audits to ensure adherence to workflows.
  • Participates as an effective member of the site Triad management team to ensure the clinical services team supports the overall operational and fiscal activities of the site to include staffing levels, budgets, financial and
    operating goals such as productivity and quality.
  • Interviews/selects candidates, approves payroll and time off requests, conducts performance evaluations, training, and orientation of new hires, and coaches/provides counseling to staff.
  • Oversees supplies inventory by developing par levels, studying usage reports, identifying trends, anticipating needed supplies, approving requisitions, and cost allocations.
  • Manages the clinical support team. Establishes work assignments, implements, and provides oversight of clinical care delivered by licensed nursing staff and medical assistants. Ensures that patient care huddles are being
    conducted.
  • Maintains positive, effective, and timely communication with partner and community organizations including but not limited to:
    • Referring partners regarding status of referral
    • Bed availability
    • Central Health partner programs regarding discharge planning
    • Skilled nursing facility referrals
    • Respite facilities regarding census updates, submitting requests for approval, and addressing mobility and infection control plans with respite facilities, etc., as indicated.
  • As needed, assists with the Respite screening and admission process to facility, by completing admission interview, educate patient regarding scope of services, respite facility guidelines, required electronic health record program documentation, and assess for DME, wound care supply needs, etc.
  • Consulting with Respite Provider leadership as needed to determine appropriateness of level of care.
  • Coordinates admission time to facility with referrer, medical respite team, transition of care team, transportation coordinator.
  • As needed, provides direct nursing care (vital signs, disease/medication education, basic wound care) to patients in a medical respite facility/clinic/community-based setting to ensure all needs of the patient are met, in conformance with established policies and procedures.
  • Manages oversight of equipment needs, equipped examination rooms, infection control standards, CLIA, compliance with Standing Delegation Orders, and emergency protocols.
  • Monitors the compliance with standards of care for on‐site treatments and procedures, administration of medications, as well as timely and quality clinical workflows.
  • Enhances the patient experience by addressing and resolving patient issues in a timely manner.
  • Ensures daily/weekly quality assurance checklists to include delegation of tasks as appropriate to staff (McKesson, CLIA, equipment checks, etc.)
  • Ensures all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards, and Central Health Standard Operating Procedures,
    Policies and Procedures. Adheres to the Texas Board of Nursing and Texas Nurse Practice Act requirements and to other governing agency regulations.
  • Participates in leadership activities such as serving on committees as assigned, participating in continuous quality improvement projects and organizational initiatives to promote and maintain a strong positive workplace
    culture.
  • Assists in the development of departmental protocols, policies, and procedures.
  • Coordinates and attends staff meetings and education offerings both in person and via teleconference/online as required.
  • Performs other duties as assigned.








Qualifications






EDUCATION:

  • Associate's Degree from an accredited school of nursing required.
  • Bachelors of Science in Nursing preferred.

WORK EXPERIENCE:

  • 2 years of clinical nursing leadership experience in a clinic, hospital, or related setting required.
  • 1 year of experience performing wound care required
  • 2 years of nursing clinical experience in an ambulatory setting, hospital, home health, or community agency preferred.
  • Experience managing populations with complex medical needs and those experiencing homelessness preferred.
  • Experience with Epic preferred.

LICENSES & CERTIFICATIONS:

  • Unrestricted Registered Nurse (RN) License in the State of Texas -OR- Compact State Licensure required upon hire. 
  • Basic Life Support certification obtained through approved American Heart Association or Red Cross required upon hire. 
  • Successful completion of a wound care certification program preferred.
