Village Green Memory Care Community is seeking a compassionate, highly skilled, and experienced Remote Nurse Practitioner (NP) to join our remote healthcare team. The Remote Nurse Practitioner will provide high-quality, patient-centered care through telehealth services, including assessments, consultations, diagnosis, and treatment plans. This role requires a strong clinical background, proficiency with telemedicine technology, and the ability to work independently in a virtual healthcare setting.
Key Responsibilities:
- Patient Care:
- Conduct virtual consultations with patients, evaluating symptoms, medical history, and current health concerns.
- Develop personalized treatment plans, including prescribing medications, ordering tests, and providing follow-up care.
- Provide health education and counseling on disease prevention, wellness, and lifestyle modifications.
- Monitor chronic conditions and coordinate care with other healthcare providers.
- Clinical Documentation:
- Accurately document all patient interactions, including assessments, diagnoses, treatment plans, and follow-up care, in accordance with healthcare regulations and policies.
- Ensure compliance with all privacy laws and maintain patient confidentiality.
- Collaboration:
- Work closely with physicians, nurses, medical assistants, and other healthcare providers to coordinate patient care.
- Consult with medical teams on complex cases when necessary.
- Telehealth Technology:
- Utilize telemedicine platforms to perform assessments, manage follow-ups, and engage with patients.
- Stay up-to-date with telemedicine technologies and healthcare trends, providing feedback on system improvements.
- Patient Support:
- Address patient questions, concerns, and provide support through phone, video calls, or messaging.
- Manage prescriptions, including electronic prescribing, medication refills, and addressing medication-related issues.
Qualifications:
- Education and Certification:
- Master's degree in Nursing (MSN) from an accredited institution.
- Current Nurse Practitioner certification (FNP, ANP, or specialty) from a nationally recognized certifying body (e.g., AANP, ANCC).
- Active Nurse Practitioner license in the state(s) where you are practicing (additional state licenses may be required depending on the role).
- Current BLS/CPR certification.
- Experience:
- Minimum of 2-3 years of clinical experience as a Nurse Practitioner (telehealth experience preferred but not required).
- Strong experience in providing primary care or specialized care depending on the role (e.g., Family Medicine, Psychiatry, Pediatrics, etc.).
- Skills and Competencies:
- Excellent communication and interpersonal skills, especially in a remote environment.
- Strong clinical assessment skills, including the ability to make sound medical decisions without direct physical examination.
- Proficiency in using electronic health records (EHR) systems and telemedicine platforms.
- Ability to work independently and manage a virtual caseload efficiently.
- Strong problem-solving skills and the ability to handle patient concerns in a compassionate and professional manner.
- Technical Requirements:
- Reliable internet connection and a quiet, private space to conduct virtual consultations.
- Comfort and experience using video conferencing tools, patient portals, and other healthcare technology.
Preferred Qualifications:
- Telemedicine experience or training.
- Experience in the specific area of care (e.g., psychiatry, pediatrics, gerontology, etc.).
- Knowledge of state-specific telehealth laws and regulations.
