Job Summary

Nurse Practitioners Services of Texas (NPST) is a Nurse Practitioner led organization which provides care to long term care residents enrolled in Texas Independence Health Plan (TIHP) an Institutional Special Needs Plan (ISNP).

Position Overview

We are seeking a skilled and motivated Nurse Practitioner to join our practice. This role will involve providing primary, chronic and acute care management to patients in long term care facilities located in (city).

The emphasis of our practice is on proactive and preemptive care through primary, chronic and acute care management, increased availability of on-site clinicians, and early identification of changes in condition to improve outcomes.

The Nurse Practitioner establishes individualized care plans for each member and facilitates communication between members, families, providers, and facility staff, and establishes advanced care preferences with each member. The NP must be versed in providing education about disease progression and addressing possible complications early in the disease process to prompt the best outcomes.

Essential Functions

The NP is responsible for provider-level care coordination, as outlined in the TIHP Model of Care, for all ISNP members.

The NP is generally expected to be in their assigned facility during customary working hours (Monday through Friday 8:00am – 5:00pm) to ensure facility has adequate provider-level coverage, while also allowing to have adequate time for documentation.

The NP shares rotational weekday/weekend and holiday on-call responsibility (phone calls only). 

  • Responsible for provider-level health care delivery and preventative care management as outlined in TIHP’s Model of Care.
  • Perform routine and acute patient assessments and physical exams.
  • Orders and interprets laboratory and diagnostic studies for acute and chronic conditions and responds to abnormalities appropriately.
  • Provides ongoing evaluation and treatment of chronic illnesses and acute conditions.
  • Reviews medications and orders monthly, and as needed, to optimize care.
  • Provides education about disease management and disease trajectory to members and their responsible parties.
  • Discusses Goals of Care (GOC), Advance Care Preferences (ACP), and anticipatory planning with members and their responsible parties.
  • Makes medical recommendations and/or referrals as needed.
  • Facilitates communication and collaboration among all care team members.
  • Provides education and in-services to increase clinical knowledge and skills of facility nursing staff.
  • Collaborates with facilities’ attending physician and Medical Director.
  • Adheres to and is accountable for all ISNP Model of Care provider metrics and goals.
  • Participates in an on-call rotation after hours, weekends and holidays.
  • Remains available to facility staff for questions, notifications, and family concerns.
  • Efficiently navigates and operates a laptop computer and enters data and documentation into designated Electronic Medical Record Systems in a timely manner.
  • Participate in Quality Improvement at assigned facilities.
  • Dresses and communicate professionally in conversations with families and staff.
  • Performs all other duties congruent with TIHP’s Model of Care, as assigned.

Educational/Training Requirements

  • Master’s degree in nursing (MSN)
  • Current certification as a Nurse Practitioner
  • Excellent interpersonal and communication skills
  • Experience in long-term care is favorable
  • Ability to work independently and as part of a team
  • Strong clinical assessment and diagnostic skills
  • New grads welcome to apply

Licensing Requirements

  • Current and valid Texas state licensure as a Registered Nurse
  • Current and valid Texas state licensure as a Nurse Practitioner (APN/APRN, FNP, AGPCNP, AGACNP, ANP, GNP)

Employment Requirements

The NP is licensed as both a Registered Nurse and an Advanced Practice Registered Nurse in the state of Texas. Bilingual (English/Spanish) is beneficial. The NP maintains an up-to-date Board Certification in his/her specialty.  The NP is responsible for maintaining a Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors and for maintaining Texas Board of Nursing ongoing educational requirements for licensure and re-certification. The NP will comply with all State and Federal guidelines required to practice as a licensed Advanced Practice Registered Nurse, including maintaining an active RN and APRN license.  The NP will maintain Professional Medical Malpractice Liability Insurance.
